Antibiotic may provide benefit for treatment of respiratory disorder

Mar. 26, 2013 ? Among patients with the lung disorder non-cystic fibrosis bronchiectasis, treatment with the antibiotic azithromycin or erythromycin resulted in improvement in symptoms but also increased the risk of antibiotic resistance, according to two studies appearing in the March 27 issue of JAMA.

Bronchiectasis is characterized by abnormal widening of the bronchi (air tubes that branch deep into the lungs) and can cause recurrent lung infections, a disabling cough, shortness of breath, and coughing up blood. "If progressive, this process may lead to respiratory failure and the need for lung transplantation or to death," according to background information in one study. Macrolide (a class of antibiotics) antibiotics have antibacterial and anti-inflammatory properties that conceivably would provide effective treatment of bronchiectasis. These antibiotics have been shown beneficial in treating cystic fibrosis (CF), and findings from small studies suggest a benefit in non-CF bronchiectasis.

Josje Altenburg, M.D., of the Medical Centre Alkmaar, the Netherlands, and colleagues conducted a multicenter trial to investigate whether 1 year of low-dose macrolide treatment added to standard therapy is effective in reducing exacerbation frequency in patients with non-CF bronchiectasis. The randomized, placebo-controlled trial was conducted between April 2008 and September 2010 in 14 hospitals in the Netherlands among 83 outpatients with non-CF bronchiectasis and 3 or more lower respiratory tract infections in the preceding year. Patients received azithromycin (250 mg daily) or placebo for 12 months.

Forty-three participants (52 percent) received azithromycin and 40 (48 percent) received placebo and were included in the modified intention-to-treat analysis. A total of 117 exacerbations treated with antibiotics were reported during 1 year of treatment, 78 of which occurred in the placebo group. "During the treatment period, the median [midpoint] number of exacerbations in the azithromycin group was 0, compared with 2 in the placebo group. Of the 40 participants receiving placebo, 32 (80 percent) had at least 1 exacerbation during the study period. In the 43 participants receiving azithromycin, 20 (46.5 percent) had at least 1 exacerbation in the same period, yielding an absolute risk reduction of 33.5 percent. The number of patients needed to treat with azithromycin to maintain clinical stability was 3.0," the authors write.

"Gastrointestinal adverse effects occurred in 40 percent of patients in the azithromycin group and in 5 percent in the placebo group but without need for discontinuation of study treatment. A macrolide resistance rate of 88 percent was noted in azithromycin-treated individuals, compared with 26 percent in the placebo group."

"We conclude that macrolide maintenance therapy was effective in reducing exacerbations in patients with non-CF bronchiectasis. In this trial, azithromycin treatment resulted in improved lung function and better quality of life but involved an increase in gastrointestinal adverse effects and high rates of macrolide resistance," the authors write.

In another study, David J. Serisier, M.B.B.S., D.M., F.R.A.C.P., of Mater Adult Hospital, South Brisbane, Australia, and colleagues tested the hypothesis that low-dose erythromycin would reduce pulmonary exacerbations in patients with non-CF bronchiectasis with a history of frequent exacerbations.

The study consisted of a 12-month randomized controlled trial of erythromycin in currently nonsmoking, adult patients with non-CF bronchiectasis with a history of 2 or more infective exacerbations in the preceding year. The study was undertaken between October 2008 and December 2011 in a university teaching hospital. Patients received twice-daily erythromycin ethylsuccinate (400 mg) or matching placebo. The primary measured outcome was the annualized average rate of protocol-defined pulmonary exacerbations (PDPEs) per patient. Secondary outcomes included macrolide resistance and lung function.

Six-hundred seventy-nine patients were screened, 117 were randomized (58 placebo, 59 erythromycin), and 107 (91.5 percent) completed the study. The researchers found that erythromycin significantly reduced PDPEs (76 for the erythromycin group vs. 114 for the placebo group; average 1.29 vs. 1.97 respectively, per patient per year). The number of patients treated with erythromycin who had zero PDPEs was 20 (vs. 16 for placebo), and 10 patients had more than 2 PDPEs (vs. 18, respectively).

Erythromycin also reduced PDPEs in the prespecified subgroup with baseline Pseudomonas aeruginosa airway infection. In addition, there were significantly fewer total respiratory events (total PDPEs plus non-PDPEs) in the erythromycin group (111 vs. 176 for placebo; average, 1.88 vs. 3.03 per patient per year).

"Erythromycin reduced 24-hour sputum production and attenuated [lessened] lung function decline compared with placebo. Erythromycin increased the proportion of macrolide-resistant oropharyngeal streptococci," the authors write.

"In conclusion, long-term low-dose erythromycin significantly reduced exacerbations, protected against lung function decline, reduced sputum production, and significantly increased macrolide resistance in oropharyngeal streptococci. The bacterial resistance caused by macrolide therapy mandates a cautious application of this therapy in clinical practice. Further studies are needed to evaluate the possibility that P aerugmosa-infected individuals with frequent exacerbations may represent an appropriate subgroup for limitation of this therapy."

U.S. victims of mass shootings seek control over donations

NEW YORK (Reuters) - Survivors of mass U.S. shootings have united to provide victims of future tragedies greater control over donations made after such events and to prevent nonprofit groups from holding onto money intended for families of the dead and wounded.

A group representing families of those killed at the Columbine, Virginia Tech and Aurora mass shootings wants to ensure any unspecified funds raised as a result of the Newtown shooting go directly to victims and their families.

Newtown, the Connecticut town where a gunman killed 20 children and six adults at Sandy Hook Elementary School on December 14, has identified more than 60 funds raising money on behalf of victims or projects related to the shooting.

The families of some mass shooting victims want a National Compassion Fund established to manage future donations.

"Going back to Oklahoma City, we've seen families who have had to endure not only horrific loss, but also the unimaginable task of wrestling with Byzantine nonprofit bureaucracies to access financial relief intended for them," the families said in a statement. "It's time to stop the madness. We cannot watch this happen, yet again, in Sandy Hook."

The informal group, so far unnamed, has initiated talks with senior White House officials and two members of Congress about establishing an official fundraising operation for such tragedies, said Caryn Kaufman, a spokeswoman for the families. She declined to name the officials out of concern it might jeopardize the project.

The group is examining whether legislation or an executive order would be needed to create a federal or nonprofit entity that would coordinate donations after any future tragedy.

The project has won initial support from Kenneth Feinberg, the influential Washington lawyer who administered funds for victims of the September 11, 2001, attacks and several mass shootings.

Jerri Jackson, whose son Matt McQuinn was killed with 11 others in the 2012 movie theater shooting in Aurora, Colorado, wants it to be easier for future victims to receive aid.

"It's not a matter of if something else happens, it's when something else happens," Jackson said. "Immediately, a fund would be set up. It would be a trusted fund that people would feel they could give to and the money would go to the victims."

Jackson said she was forced to "jump through hoops in the midst of tragedy" to receive money raised in her son's name.

Nearly $5.9 million in Aurora donations were channeled to the Community First Foundation. When victims' families publicly complained about the pace of disbursement, Colorado Governor John Hickenlooper called in Feinberg, who oversaw the disbursement within 45 days. The victims' fund was later shut down.

After the 2007 Virginia Tech University shooting that killed 32 people, more than $10 million spontaneously flowed to the university's fundraising foundation, university spokesman Larry Hincker said.

Because that foundation was only authorized to spend money on the university, an executive order from the governor was needed to disburse the funds to victims, Hincker said.

Michael Pohle, a member of the group whose son was killed at Virginia Tech, said: "What was so insulting was we had to fill out documents, have them notarized, and basically beg and apply for dollars."

Feinberg recognized some grieving survivors were upset but defended the disbursement process following both tragedies, noting the highly unusual circumstances, complex legal requirements and emotional strain on the families.

FAMILIES BAND TOGETHER

The ad hoc group of 64 people lost relatives in tragedies such as the September 11 attacks, the 2008 shooting at Northern Illinois University and the Sikh temple shooting last year at Oak Creek, Wisconsin.

The group has not involved the Sandy Hook families regarding the distribution of Newtown funds, saying it was too soon for those still grieving. Even so, the group was concerned about the confusion caused by having dozens of nonprofit groups collecting money in Newtown, said Kaufman, the group's spokeswoman.

Some of the Newtown funds were designated for a specific purpose, such as creating a playground or scholarship. In other cases, the funds refer to more general goals of supporting the victims. The group believes that money should go to the victims' families.

Feinberg has overseen disbursements of funds following September 11, the BP oil spill and several mass shootings. Though he is not affiliated with the group, he supports the concept Of having a protocol in place when a future tragedy occurs.

"I think it's a great idea," Feinberg said. "The question is: Is there the political will to do it?"

Troubled Calif. nuke plant seeks restart in summer

LOS ANGELES (AP) ? As part of an effort to convince federal regulators that a nuclear reactor is safe to restart, the operator of the shuttered San Onofre nuclear power plant in California has disclosed it might push for a rewrite of the facility's operating rules.

Southern California Edison disclosed Friday it hopes the move could open the way for the Unit 2 reactor to be back in service by summer, when power demand typically soars in the region.

San Onofre has been shut down since January 2012, after a small radiation leak led to the discovery of unusual damage to hundreds of tubes that carry radioactive water.

Edison has been trying since October to convince the Nuclear Regulatory Commission that it's safe to run Unit 2 at no more than 70 percent power. Company engineers believe the reduced level will limit vibration and friction that can cause excessive wear to tubing.

The tentative proposal amounts to Edison's third attempt to answer a thorny question raised by the NRC: Is the plant that hasn't produced electricity in more than a year capable of running at full power?

In earlier filings, Edison argued that its 70 percent restart target was, in effect, full power. It later submitted another analysis showing the reactor could run at 100 percent power, but the research found the risk of a tube break could reach unacceptable levels after 11 months.

The new proposal could essentially eliminate the debate over the full power threshold.

It calls for capping the plant's power output at 70 percent in the plant's technical operating rules, rather than the now-required 100 percent. It also argues that running the reactor at 70 percent capacity would pose no significant safety risk.

The proposal, known as a license amendment, came as a surprise since Edison has long argued such a revision was unnecessary to restart the plant.

If approved by federal regulators, the move could offer a potentially quicker way to a restart.

"We want to do every responsible thing we can do to get Unit 2 up and running safely before the summer heat hits our region," SCE President Ron Litzinger said in a statement.

Anti-nuclear activists who have opposed the restart accused Edison of trying to circumvent a thorough NRC review of machinery with a history of trouble.

According to Edison documents, members of the public can request a hearing on the amendment, but if NRC staff finds there is no significant hazard, the hearing can be held after the amendment is approved.

"Edison is more focused on making profits than it is in assuring the safety of millions of Southern Californians living near these reactors," Damon Moglen of the advocacy group Friends of the Earth said in a statement.

Daniel Hirsch, a lecturer on nuclear policy at the University of California, Santa Cruz, and a critic of the nuclear power industry, said Edison was trying to delay a substantive review until "long after it has already restarted."

"If it is subsequently determined it wasn't safe to do so, it would be way too late," Hirsch said in a statement.

NRC spokesman Victor Dricks said the agency had not received the proposal from Edison.

The problems at San Onofre focus on its steam generators, which were installed in a $670 million overhaul in 2009 and 2010.

Last year, federal regulators blamed heavy tube wear in the generators on a botched computer analysis that they said misjudged how water and steam would flow in the reactors, along with manufacturing problems.

The generators, which resemble massive steel fire hydrants, control heat in the reactors and operate somewhat like a car radiator. At San Onofre, each one stands 65 feet high and weighs 1.3 million pounds, with 9,727 U-shaped tubes inside that are each 0.75 inch in diameter.

Overall, NRC records show investigators found wear from friction and vibration in 15,000 places, in varying degrees, in 3,401 tubes inside the plant's four generators, two in each reactor.

The future of the heavily damaged Unit 3 reactor, where the radiation leak occurred after a tube break last year, is not clear. Edison has said that because of manufacturing differences, Unit 2's generators did not suffer the extent of deep tube wear witnessed in its sister.

Cracked and corroded generator tubing has vexed the nation's nuclear industry for years.

Decaying generator tubes helped push San Onofre's Unit 1 reactor into retirement in 1992, even though it was designed to run until 2004. The following year, the Trojan nuclear plant, near Portland, Ore., was shuttered because of microscopic cracks in steam generator tubes, cutting years off its expected lifespan.

San Onofre is owned by SCE, San Diego Gas & Electric and the city of Riverside.

Kerry warns Iraq on Iran flights to Syria

BAGHDAD (AP) ? Just days after the 10th anniversary of the U.S. invasion of Iraq, U.S. Secretary of State John Kerry confronted Baghdad for continuing to grant Iran access to its airspace and said Iraq's behavior was raising questions about its reliability as a partner.

Speaking to reporters during a previously unannounced trip to Baghdad, Kerry said that he and Iraqi Prime Minister Nouri al-Maliki had engaged in "a very spirited discussion" on the Iranian flights, which U.S. officials believe are ferrying weapons and fighters intended for the embattled Syrian government.

Kerry said the plane shipments ? along with material being trucked across Iraqi territory from Iran to Syria ? were helping President Bashar Assad's regime cling to power by increasing their ability to strike at Syrian rebels and opposition figures demanding Assad's ouster.

"I made it very clear that for those of us who are engaged in an effort to see President Assad step down and to see a democratic process take hold ... anything that supports President Assad is problematic," Kerry said at a news conference at the U.S. Embassy in Baghdad after meeting separately with Maliki at his office. "And I made it very clear to the Prime Minister that the overflights from Iran are, in fact, helping to sustain President Assad and his regime."

The overflights in Iraq have long been a source of contention between the U.S. and Iraq. Iraq and Iran claim the flights are carrying humanitarian goods, but American officials say they are confident that the planes are being used to arm the support the Assad regime. The administration is warning Iraq that unless action is taken, Iraq will be excluded from the international discussion about Syria's political future.

U.S. officials say that in the absence of a complete ban on flights, Washington would at least like the planes to land and be inspected in Iraq to ensure that they are carrying humanitarian supplies. Former Secretary of State Hillary Rodham Clinton secured a pledge from Iraq to inspect the flights last year, but since then only two aircraft have been checked by Iraqi authorities, according to U.S. officials.

One senior U.S. official traveling with Kerry said the sheer number of overflights, which occur "close to daily," along with shipments trucked to Syria from Iran through Iraq, was inconsistent with claims they are only carrying humanitarian supplies. The official said it was in Iraq's interest to prevent the situation in Syria from deteriorating further, particularly as there are fears that al-Qaida-linked extremists may gain a foothold in the country as the Assad regime falters.

The official, who spoke on condition of anonymity because the official was not authorized to speak publicly on the matter, said there are clear links between al-Qaida linked extremists operating in Syria and militants who are also carrying out terrorist attacks in Iraqi territory with increasing regularity.

Kerry's comments in Baghdad come as U.S. lawmakers are calling for President Barack Obama to do more to stop the bloodshed in Syria, including possible airstrikes against Assad's aircraft fleet.

The Republican chairman of the House Intelligence Committee, Rep. Mike Rogers of Michigan, said Sunday the U.S. should create a "safe zone" in northern Syria that would give the U.S. more leverage with opposition forces.

"This doesn't mean the 101st Airborne Division and ships" are deployed, Rogers told CBS' "Face the Nation." ''It means small groups with special capabilities reengaging the opposition so we can vet them, train them, equip them so they can be an effective fighting force."

Last week, Sens. Carl Levin, D-Mich., and John McCain, R-Ariz., asked Obama in a letter to step up U.S. military efforts in the region, including destroying Assad's aircraft using precision airstrikes.

Kerry said Iraq's tacit approval of Iranian overflights left the American people wondering how an ally would undermine U.S. efforts, particularly after the enormous sacrifices made by the United States in liberating Iraq from Saddam Hussein's tyrannical rule.

"There are members of Congress and people in America who increasingly are watching what Iraq is doing and wondering how it is that a partner in the efforts for democracy and a partner for whom Americans feel they have tried so hard to be helpful, how that country can be, in fact, doing something that makes it more difficult to achieve our common goals, the goal expressed by the prime minister with respect to Syria and President Assad," he said.

In addition to the overflights, Kerry said he had urged Maliki and other Iraqi officials to promote unity amid a spike in sectarian violence and called on them to ensure that upcoming provincial elections are free and fair. Kerry said the postponement of the polls in two provinces ? Anbar and Ninevah ? was unacceptable and should be reversed.

"We strongly urge the prime minister to take this issue to the cabinet and to see if it can be revisited, because we believe very strongly that everybody needs to vote simultaneously," he said.

In addition to his meeting with Maliki, Kerry saw Iraqi parliament speaker parliament speaker Osama al-Nujaifi, a Sunni, whose faction is at odds with Maliki's Shiia. Kerry also spoke by phone with Massoud Barzani, the head of the Kurdish Regional Government based in Irbil to encourage the Kurds not go ahead with unilateral actions ? especially involving oil, like a pipeline deal with Turkey.

Kerry arrived in Baghdad from Amman, where he had been accompanying President Barack Obama on his tour of Israel, the Palestinian Authority and Jordan. His visit to Iraq is the first by a U.S. secretary of state since Clinton went in April 2009. During Obama's first term, the Iraq portfolio was largely delegated to Vice President Joe Biden as Obama wound down the war.

What Is a Vote-A-Rama?

Senate Majority Leader Harry Reid is getting ready for a vote-a-rama this week. That sounds like?an old, tired?SNL?skit, but don't be fooled -- it's actually an old, tired Senate trick.

Well, not that tired. Here's the story: Sometimes, the Senate holds flurries of votes on budget resolutions. Debate on budget reconciliation bills is limited to 20 hours, and the resolutions can't be filibustered, so the only way to draw the process out is to offer amendments. This being the Senate, the opposition seldom misses the opportunity to do so. Once debate has ended, the amendments come in rapid fire. There could be literally dozens of amendments (there are often at least 20 or 30), but the time spent on them is minimal. In theory, there's not even time to debate, though as Keith Hennessey -- who has?a great primer on the topic?-- points out, they often waive the rules and allow each side gets a grand total of 30 seconds to debate, followed by a 10-minute vote. Then the next, and the next, and so on until everything is finished and everyone can go home for the night.

This makes for a high-pressure environment. You often hear that legislators don't read the bills in front of them under normal circumstances, but that's in part because staffers are supposed to analyze the language and tell their bosses how to vote. In this case, with amendments coming up so quickly, it's harder to do that. Senators havecomplained?that it's essentially impossible to tell what's going on, but staffers huddle in the party cloakrooms to try to suss it out as the votes are happening. The vote-a-rama could run for 10 hours or more, and the exhaustion is compounded by the fact that it comes at almost the end of a fractious budget process.

So why would this ever happen? They are infrequent; the last one wasnearly three years ago, and?many current senators have?never experienced?one. One reason for holding a vote-a-rama is to get things done quickly. Because of?filibusters, holds, and other procedural tricks, much of the Senate's business has been reduced to a crawl. This allows Reid to get a lot done quickly. But there's also a chance for political grandstanding. The?Wall Street Journal?reports, for example, that Democrats will bring Paul Ryan's budget to the floor as an amendment to their own. They know they have the votes to defeat the amendment, so they want to force GOP senators to either vote for the bill, and risk being tarred as too conservative, or vote against it, and risk the appearance of divisions in the Republican ranks. Republicans, meanwhile, will?offer?amendments to do things like balance the budget within a short timeframe or oppose tax increases, making Democrats vote against them.

The timing for the vote-a-rama isn't clear yet, but it will start no later than 7 p.m. Friday. As for when it ends ... well, it could get late. Don't wait up for them.

Most pre-packaged meals, snacks for toddlers contain too much salt

Mar. 21, 2013 ? Nearly 75 percent of commercial pre-packaged meals and savory snacks for toddlers are high in sodium, according to research presented at the American Heart Association's Epidemiology and Prevention/Nutrition, Physical Activity and Metabolism 2013 Scientific Sessions.

In the first study to look at the sodium content in U.S. baby and toddler foods, researchers compared the sodium content per serving of 1,115 products for babies and toddlers using data on major and private label brands compiled by the U.S. Centers for Disease Control (CDC). Baby food was categorized as intended for children less than one year old, and toddler food was categorized as intended for children between the ages of one and three.

A product was defined as high in sodium if it had more than 210 mg of sodium per serving. Toddler meals had significantly higher amounts of sodium than baby meals, and the amount of sodium in some of the toddler meals was as high as 630 mg per serving -- about 40 percent of the 1,500 mg daily limit recommended by the American Heart Association. The foods with the most sodium were savory snacks and meals for toddlers.

"Our concern is the possible long-term health risks of introducing high levels of sodium in a child's diet, because high blood pressure, as well as a preference for salty foods may develop early in life. The less sodium in an infant's or toddler's diet, the less he or she may want it when older," said Joyce Maalouf, M.S., M.P.H., ORISE, lead author and Fellow at the National Center for Chronic Disease Prevention and Health Promotion at the Centers for Disease Control and Prevention in Atlanta, Georgia. Consuming excessive amounts of sodium has been linked to the development of high blood pressure in scientific studies.

"Parents and other caregivers can read the nutrition facts labels on baby and toddler foods, to choose the healthiest options for their child," Maalouf said.

The American Heart Association recommends limiting sodium consumption to less than 1500 mg a day. Sodium is in regular table salt and many foods, including most prepared meals and snacks for toddlers.

Chinua Achebe, the Nigerian novelist and poet whose 1958 novel "Things Fall Apart" addressed the effects of colonialism on African society, has died. He was 82.

Achebe died following a brief illness, his literary agent, Andrew Wylie, said Friday. He added that the author's family has requested privacy.


Brown University, where Achebe taught for the past four years, said the writer passed away Thursday night.

"We join the world in mourning Chinua Achebe's death," John Makinson, chairman and chief executive of Penguin Group, said in a statement. "He was a giant, and a wise and kind man. We are honored to have published his final book, which, like all of his books, will outlast us all."

Achebe?s breakthrough novel focused on the clash between Western and traditional values. It told the story of colonialism for the first time from an African perspective, and?has sold more than 10 million copies and been translated into more than 50 languages.

Nelson Mandela has credited Achebe for bringing "Africa to the rest of the world" and called him "the writer in whose company the prison walls came down."

Achebe played a pivotal role in the development of African literature.?He is the author of more than 20 books, including novels, short stories, essays and poetry collections. In 2007, he won the Man Booker International Prize for Fiction.

Achebe was born Nov. 16, 1930 and raised in the village of Ogidi in southeastern Nigeria. After graduating from college, he started a radio career that ended?abruptly?in 1966 during the national upheaval that led to Nigeria?s three-year civil conflict, known as the Biafran War. Achebe then joined the Biafran Ministry of Information and represented Biafra on various diplomatic and fund-raising missions.

He was known as an outspoken critic of successive dictatorship governments in Nigeria, often refusing to accept literary honors from his home country in protest.

Last year, Achebe published his final book, "There Was a Country," a memoir about the three-year Biafran War.

Russell Perreault, publicity director for the writer?s earlier publisher, Random House, noted Achebe once said, "If you don't like someone's story, write your own."

"We are saddened by the death of the 'Father of African literature' Chinua Achebe," Perreault said. "We are grateful that he told his story and left us with a legacy of great literature and a better understanding of Africa."

At the time of his death, Achebe was a professor of Africana studies at Brown University. Among his activities at the school was the annual Achebe Colloquium on Africa, an international gathering of scholars, policymakers, and others who shared interest in current day African affairs.

"He was more than just a colleague, faculty member, and teacher at Brown. He was a gift to the world," said Corey D.B. Walker, chair of Brown?s Department of Africana studies.?"At a time like this we could draw many words of wisdom and comfort from the deep wells of various African cultures and traditions to honor him. The most fitting is the simple and elegant phrase, 'A great tree has fallen.'"

Prior to Brown, Achebe had taught for more than 15 years at Bard College. He moved there shortly after a car accident in 1990 left him paralyzed from the waist down.

He and his wife, Christie Okoli, had four children.

Blogging My Book Tour: Stops 4 and 5, Westmont College | Kelly ...

The Burn This House road trip continues its juggernaut down the California coastline! Yesterday, I arrived in Santa Barbara, where I spent several years as an undergraduate at Westmont College. I was honored to be invited back to Westmont this week to read from my collection and to guest lecture in a creative writing workshop.

I hadn?t been back to the Westmont campus since I graduated a good decade ago, and as I drove up into the hills above Santa Barbara, I had to laugh at myself for my relative inability to navigate the roads I?d spent so much time driving in my youth. Being back on campus was also an exercise in mental astonishment?everyone walking around so young. And they all looked exactly the way we students did years ago. It was as though I?d entered some strange wormhole in which I was the only one who?d aged. The campus, however, was astonishingly beautiful?a number of new, environmentally friendly and darned cool buildings have popped up in the wake of the devastating fire that swept the campus a few years ago, and it was good to see the campus not only restored but made even more beautiful than it was before.

The reading I gave was, I think, a success. At every stop along the road, I?ve been frightened that no one would come. Or that one person and/or a feral cat would come. Somehow, one person or a cat

would be?worse than no one. And when I arrived to see a huge number of chairs set up in a hall where I?d only seen Dana Gioia pack out the venue, I was not a little scared about my prospects. Happily, a great number of students, faculty, and community members came out for the reading and all but filled the hall. The reading opened with two wonderful student readers, Paul and Riley. I was not writing poems half as strong as they when I was an undergraduate, and I hope both of them continue to develop their craft.

For my own part, I read a large quantity of poems, trying to intersperse some of the funny ones with the serious. I felt that the poems were well received, and it felt wonderful to have the support of some very dear friends and teachers in the room with me. I know many of them are extremely busy this time of year, and I was honored and humbled that they made time to attend.

This morning, I visited one of the fiction writing classes at Westmont to speak with the students about how to begin placing work in literary journals. I brought them a big stack of magazines I brought back from AWP, and made sure the students took home journals to read. We talked about finding venues that specialize in particular types and lengths of fiction, about how exactly one goes about submitting work, and how to handle rejection. The students were a delight?serious, enthusiastic, open to what I had to say. I hope that if they gleaned one thing from my talk, it?s that rejection is okay. Rejection is simply a feature of the writing life, and that we should embrace rejection as a sign that we?re doing what we need to do as writers: sending our work into the world.

After a great morning at Westmont, I headed down to Los Angeles for the last stop on the California leg of this tour. This drive was a bit more grim than the others thus far on the trip, with stop-and-go traffic the entire way from Santa Barbara to Long Beach, where I?m staying.

A few points of interest along the way to Los Angeles:

1. I watched the Parent of the Year drive past me on the 405 in Los Angeles today. Said parent was on a motorcycle, threading the snug needle between the two left lanes, with an infant strapped to her back in a Baby Bjorn. I don?t know if that?s illegal, but it should be.

2. There is sure lot of flotsam and jetsam strewn around the sides of the roads here. I suppose I don?t generally notice it when I?m breezing down the freeway doing 70, but stopped on the road today, I saw enough broken glass, smashed beer cans and general detritus to make a really bad art exhibit. What are you all doing down here?

3. In ?N Out doesn?t get old on road trips. It just doesn?t.

Now, finally, after an epic day, off to get some rest.

I hope I?ll see some of you, who have not traveled with infants strapped to your motorbikes, at the Ruskin Art Club this Sunday, 2:00 pm. It should be a fabulous program! I?m delighted to be reading with a great group of other Red Hen poets.

(AP) ? America's lower-income workers have posted the biggest job gains since the deep 2007-09 recession ? but few are bragging.

As a workforce sector, those earning $35,000 or less annually are generally pessimistic about their finances and career prospects. Many see themselves as worse off now than during the recession, a two-part Associated Press-NORC Center for Public Affairs Research survey of workers and employers shows.

The survey revealed that many people at the lowest rung in the workplace view their jobs as a dead end. Half were "not too" or "not at all" confident that their jobs would help them achieve long-term career goals. And only 41 percent of workers at the same place for more than a decade reported ever receiving a promotion.

Yet 44 percent of employers surveyed said it's hard to recruit people with appropriate skills or experiences to do lower-wage jobs, particularly in manufacturing (54 percent). While 88 percent of employers said they were investing in training and education for employee advancement, awareness and use of such programs among the lower-wage workers was only modest.

Although President Barack Obama made it a national goal to "equip our citizens with the skills and training" to compete for good jobs, the survey shows a U.S. workforce that has grown increasingly polarized, with workers and their bosses seeing many things differently.

Seventy-two percent of employers at big companies and 58 percent at small ones say there is a "great deal" or "some" opportunity for worker advancement. But, asked the same question, 67 percent of all low-wage workers said they saw "a little" or "no opportunity" at their jobs for advancement.

Through last month, the economy had recovered only about 5.7 million of the 8.7 million jobs shed in the deepest downturn since the Great Depression. Low-wage jobs are usually the first to come back following a recession. While the outlook clearly is improving, economic growth remains anemic and unemployment is a still-high 7.7 percent.

Ronald Moore, 48, of Lebanon, Ind., is among those who have seen their situation improve. He started his own home-inspection company three years ago after he couldn't find enough work as a truck driver. But "nobody was buying homes, so no one needed an inspection," he said. "It was pretty rough in the beginning." Now he operates a custom cabinet business, where business is starting to improve. Slowly.

To gauge the experiences and perspectives of lower-wage workers, the AP-NORC Center conducted two separate surveys. A sample of 1,606 workers earning $35,000 or less annually was surveyed last summer, while a companion poll of 1,487 employers of such workers was conducted from November through January.

Roughly 65 percent of the jobs the U.S. economy added since the recession officially ended in June 2009 have been lower-wage ones.

Despite those numerical gains, "lower-income households have been hit very hard and have not benefited as much from the recovery," said Mark Zandi, chief economist at Moody's Analytics. "Their real wages are going nowhere. And this is a group that has more debt, fewer assets, is less likely to own a home or stocks and with little capacity to absorb higher gasoline prices."

Economists also say low-wage workers were hit particularly hard by an increase in Social Security payroll taxes resulting from "fiscal cliff" negotiations late last year between Obama and Congress.

A degree of economic "self-righting" will happen as more middle-income and higher-income jobs come back and economic growth accelerates, said Robert Trumble, director of the Labor Studies Center at Virginia Commonwealth University. "But the situation we've been facing for the last half-dozen years or so has been tough. And the lower your income, the tougher it is."

"Some things are better. But there are still some things that are still hard," said Sarah Mueller, 33, of Palm Harbor, Fla., who found work as a Montessori teacher two years ago after working as a part-time and substitute teacher. "With student loans, people are still struggling ? I'm one of those people ? to pay back student loans that are astronomical," she said.

Seventy-four percent of lower-wage workers say it is "difficult" or "very difficult" for them and their families to get ahead financially. Half thought their financial situation was somewhat or much worse than in 2008.

Many worry a lot or some (71 percent) about being unable to pay their bills, unexpected medical expenses (70 percent), losing their job (54 percent) or keeping up with their mortgage or rent (53 percent).

Many reported stagnant (44 percent) or declining (20 percent) wages over the past five years.

Employers and workers tend to agree that employees themselves hold the bulk of the responsibility for helping workers to get ahead in their careers, but employers are more apt to place some of that responsibility on high schools and colleges.

Despite their many frustrations, 74 percent of low-income workers said they were very or somewhat satisfied with their jobs. Yet 90 percent of all workers said they were satisfied with their job, according to an AP-GfK poll conducted in September.

The surge in low-wage jobs seems to have escaped notice by employers, the survey suggests. Just 22 percent of them said their organization's lower-wage workforce grew over the last four years and only 34 percent expect it to increase in the coming four years.

Lower-wage workers are also pessimistic about the overall direction of the country, with 7 in 10 saying "wrong direction," above the 60 percent of all adults who said so in AP-GfK polling conducted at the same time.

"Lower-wage jobs are coming back first," said labor economist Heidi Shierholz of the Economic Policy Institute, a labor-leaning think tank. "But it's all bleak and it's all due to lack of demand for work to be done. We're still not getting more than just what we need to hang on," Shierholz said. "These last few months have looked better, but we cannot yet claim robust recovery by any stretch."

Lena Hughes, 31, of Indianapolis, a certified hospital nursing assistant, would agree.

"Everybody is struggling financially. It's hard to get jobs still," she said. "I don't think it's getting any better."

The surveys were sponsored by the Joyce Foundation, the Hitachi Foundation and NORC at the University of Chicago. The Joyce Foundation works to improve workforce development and education systems to assist job seekers who may lack skills or credentials. The Hitachi Foundation aims to expand business practices that improve economic opportunities for less well-off workers while benefiting business.

The worker survey was conducted online using the GfK KnowledgePanel and by telephone by interviewers from NORC from Aug. 1 through Sept. 6, 2012. The employer survey was conducted online and by phone by NORC from Nov. 12, 2012, through Jan. 31, 2013. The margin of sampling error for the survey of workers was plus or minus 2.9 percentage points; for employers, it was 4.5 points.

Step-parenting is sometimes harder than I thought it would be. People on the outside seem to think I have this shit all together- when in reality I don't.

In reality I go to therapy every other week to work on myself to?in-turn?work on my relationship with the girls.

I think I thought when coming in this situation it would all just flow. We would be this happy family of 4- the end. Not! When I think about all the steps and methods we have been through since day one to get to where we are today, it's a lot. I am also unsure I could ever go through it all again.

Olivia, Gracie, and I have this unspoken communication for our situation. The three of us get it. We know what we are to each other and that is all that matters. I am their Lindsay. They are My Olivia. My Gracie. Other than that I am unsure if any of us could actually put what our relationship is into words. I am usually caught off guard by their feelings for me. I am here everyday and it still?surprises?the hell out of me each and?every time?they bring home a picture or a poem for me.

My only expectation is they respect me. They don't have to love me or like me or want to be around me, but respect is important. I think we are all learning that.

I have spent the better part of my twenties trying to be a good wife, a good parent, and create a family unit. That?surprises?even me. I never thought I would ever be where I am- ever. I recently tried to get into the HBO series Girls and I just don't get it. I think it's because at 22 my life stopped being like theirs and I have spent the last 5 years thinking about myself last. It's interesting how that all just falls into place- like it was always supposed to be.

I also find it interesting when people feel the need to remind me that Olivia and Gracie have a Mother. Mostly because, I know that! Do people honestly think I don't know that? Just because these girls didn't fall out of my vag doesn't me I don't have the right to love them. Love them I do- with everything I have in me.

I think that is what?surprises?me the most, is my never ending love for them. I could really not like them in moments, but love always brings it full circle. I would do anything for them if I knew it would make their life better- anything.

Kelly has never once pushed the three of us together. He never pushed for more than what we have in us. It amazes me that he could just watch. He is always there for all three of us in just the right ways even when we are trying to all be against each other. I feel sorry for what the teen years brings him.

There was never a book that fit our situation. Believe me we looked. So when people ask me how it all works and to give advice to them- I don't really have any. Nor do I feel qualified to give any. We will always be a loving work in progress. You have to do what works for YOUR family. Not based around what a book says or what the world thinks. Honestly, with LOVE you can do anything. Thank God for love because we don't always like each other.

Today's Video Newsletter: 3 New Stocks Join the Bull Parade

With the FED pouring $4.25 billion into the system every trading day, it's no wonder that stocks are moving higher. When this artificial demand and false sense of security stops, what will happen to the markets? No one can tell for sure, but we cannot keep pumping money into this market forever. Yesterday, Federal Reserve Bank of Philadelphia President, Charles Plosser, said ?We should begin to taper our asset purchases with an aim of ending them before year-end" and he believes that the central bank should slow the pace of its bond purchases because the potential costs from more stimulus outweigh the benefits.

When the FED turns off the money spigot, watch out!! That's when we get to see if the real economy has any clothes on. Until then, happy days are here again and the bull bandwagon is moving higher courtesy of the FED.

We are going to be looking at three stocks that jumped on the bull band wagon yesterday, they are:

GAP (GPS) - The technical trend is positive for this stock. All of our Trade Triangles are green. We see this market moving up to $36.85-$37, where it should run into previous levels of resistance. Present trading level around $35.50. Continue to monitor the Trade Triangles for early exit signals.

PAYCHEX (PAYX) - The technical trend is positive for this stock. All of our Trade Triangles are green. We see this market moving up to the $37-$38 range from its present trading level around $34. Continue to monitor the Trade Triangles for early exit signals.

VERTEX (VRTX) - The technical trend is positive for this stock. All of our Trade Triangles are green. We see this market moving up to the $52-$53 range from its present trading level around $49.50. Continue to monitor the Trade Triangles for early exit signals.

House, debris over Fla. sinkhole to be removed

SEFFNER, Fla. (AP) ? The gaping sinkhole that swallowed a man from his Florida home last week could be revealed later Monday when demolition crews knock down the remaining walls and begin clearing away the debris.

Crews on Sunday razed more than half the home, managing to salvage some keepsakes for family members who lived there.

The opening of the sinkhole has been covered by the home, but once emergency officials and engineers can see inside it more clearly, they could begin planning how to deal with it. They also need to decide what will happen to the two homes on either side of the now-demolished house. Experts say the sinkhole has "compromised" those homes, but it's unclear whether steps can be taken to save them.

Jeremy Bush, 35, tried to save his brother, Jeff, when the earth opened up and swallowed him Thursday night.

On Sunday morning, Bush and relatives prayed with a pastor as the home ? where he lived with his girlfriend, Rachel Wicker; their daughter, Hannah, 2; and others ? was demolished and waited for firefighters to salvage anything possible from inside. The home was owned by Leland Wicker, Rachel's grandfather, since the 1970s.

The operator of the heavy equipment worked gingerly, first taking off a front wall. Family belongings were scooped onto the lawn gently in hopes of salvaging parts of the family's 40-year history in the home.

As of Sunday afternoon ? when demolition had stopped for the day and only a few walls remained ? a Bible, family photos, a jewelry box and a pink teddy bear for Hannah were among the items saved. Firefighters also were able to pick out the purse of one of the women in the home.

Cheers went up from family, friends and neighbors each time something valuable was salvaged.

Wanda Carter, the daughter of Leland Wicker, cradled the large family Bible in her arms. She said her mother and father had stored baptism certificates, cards and photos between the pages of that Bible over the years.

"It means that God is still in control, and He knew we needed this for closure," she said, crying.

Carter said she spent from age 11 to 20 in the home, and she had to close her eyes as the home was knocked down.

"Thank you for all of the memories and life it gave us," she said.

The Rev. John Martin Bell of Shoals Baptist Church said he had been with the family all morning. "We just prayed with them," he said. He added that all five who lived in the house ? Bush, Wicker, Hannah and two others ages 50 and 45 ? were in need of support and prayers from the community.

Several generations of family members lived in the home at the time of the ground collapse, including Jeff Bush, the man now presumed dead.

Jeremy Bush tried to save his brother by jumping into the sinking dirt hole. He had to be pulled out of the still-shifting hole by a Hillsborough County Sheriff's deputy, who was visibly shaken when talking about the incident more than a day later.

"I've never seen anything move so fast and do so much destruction," Deputy Douglas Duvall said.

The search for Jeff Bush, 37, was called off Saturday. He was in his bedroom Thursday night in Seffner ? a suburb of 8,000 people 15 miles east of downtown Tampa ? when the ground opened and took him and everything else in his room. Five others in the house at the time escape unharmed as the earth crumbled.

The Hillsborough County Sheriff's Office is conducting the investigation. Detective Larry McKinnon said the sheriff's office and the county medical examiner cannot declare Bush dead if his body is still missing. Under Florida law, Bush's family must petition a court to declare him deceased.

"Based on the circumstances, he's presumed dead; however the official death certificate can only be issued by a judge and the family has to petition the court," McKinnon said.

The area around Seffner is known for sinkholes due to the geography of the terrain, but they are rarely deadly. No one ? from longtime public safety officials to geologists ? could remember an incident where a person was sucked into the earth without warning.

Facebook Redesign - Business Insider

Two months ago, an Australian designer named Fred Nerby redesigned Facebook as a side project for himself and posted it to Behance.

His design went nuclear. Nerby says at minimum it's received 20 million views worldwide.?

With such notice comes some perks.

Just twenty-four hours after his design went live, he says firms from around the world started reaching out to him. "I honest lost count after a couple of days," Nerby tells us over email. "Things changed drastically for me and I got invited to spend time overseas talking to a large number of companies and creatives within the digital space, both agencies and more product/platform based companies in the California region (Facebook being one of them) and also Australia, which has been fantastic!"

He wouldn't reveal much about his conversations with Facebook, other than he's not going to be working there. Instead, he says he took a job with a different firm, though he won't reveal where.

The lesson here: If you do good work, and it gets noticed, you can end up with a sweet new gig. It is a little bit of a needle in a haystack, but it's possible.

Motivation, some of us are born with this attribute. While?for others it is learned, from?a parent, family member, friend or maybe from a book. Motivation or what we believe to be motivation, is subjective especially when we are evaluating ourselves. To be critical of ourselves is not easy and lets face it admitting to oneself ?we are lax? or have been approaching some aspect of our lives wrong? How many of us can actually say we have evaluated ourselves honestly??Self improvement does not need to be?a horrifying journey of reflection, action and attempted change. Improving ourselves and achieving lasting improvement, all about opening our minds to new ideas/sources.

Finding the right source, to suggest one specifically would imply that we all are the same which we are not. With plenty of material out there to choose, from Dr. Phil to old school sources like Norman Vincent Peale?s ? The Power of Positive Thinking?, the self-improvement resource part is rather easy. Getting through that resource and learning from it, that?s the hard part! For the love of books, I find myself most often at the used book store and thrift stores. Pricing has something to do with it sure but the search for a surprise?good read is what I love about these stores the most. Nothing in mind when I go there so if I find a book, I buy it, if I don?t I leave empty-handed.?The most recent purchase I made?for $1.00, Pat Croces? ?I Feel Great and you will too? published in 2000.?The book about his journey of success and life story on how he achieved success. Owner of Physical Therapy Centers around the country he was also the former owner of the Philadelphia Seventy Sixers. Being from the area I? knew something about him, I am a Sixer fan so ?why not?. (Note: was not in the market or belief I needed motivation)

The book was?a relaxing, funny read and also as stated in the title ??.and you will too!? Feeling Great and saying it, a simple philosophy which was reinforced throughout the entire book. As I stated I was not in the belief I needed or was in the market for a motivational book but after reading it, I DID NEED IT! How we think most certainly impacts everything we do and how well we do it. I plan on keeping this book even if only to look at the title as a reminder. Searching for motivation or changing how we approach our lives, if we task ourselves to do this I believe makes it?far more difficult. Nothing against Dr. Phil, ?Norman Vincent Peale?or the many others who write self-improvement, but motivation and becoming motivated can be learned/improved a heck of a lot easier when we aren?t looking for it.
